Supplemental Health Insurance
Supplemental health insurance plans (indemnity insurance), like AFLAC, offer limited benefit insurance coverage that may help you cover your out-of-pocket medical expenses. These plans normally pay you directly. Because they are not covered under the HIPAA transaction rules, Brookings Health System cannot directly file a claim nor send your medical record directly to your supplemental insurance provider. This follows HIPAA privacy laws that protect your personal health information.
We want to work with you and your supplemental insurance provider to process your claim as quickly as possible. To expedite your payment from your supplemental health insurance provider, you will need to either sign an authorization to release claims for supplemental or indemnity insurance or enroll in the MyHealth Patient Portal. You may choose to share medical record information from the portal with your supplemental health insurance provider.
Completed forms must be returned either in person, by mail or by fax to Brookings Health System. We will send requested medical records to the address listed on the completed form.
